Turkish Kebab and Pizza House, a takeaway at 112 Andersonstown Road, Belfast was given the score after assessment on June 29, the Food Standards Agency's website shows.
It means that of Belfast's 403 takeaways with ratings, 212 (53%) have ratings of five and none have zero ratings.
